Whole & Healthy Church Policies

These Policies were written and approved by Executive Council in 2017, combining the former policies of the Diocese with the Model Policies of The Episcopal Church.

Whole & Healthy Church Acknowledgement Letter

All Clergy canonically resident in the Diocese of California are required to read the policies, initial the agreements on this letter, and return it to the Diocesan Office.

Participant and Volunteer Behavior Agreement

This is a one-page document for volunteers and participants to read and sign in agreement to policies, practices, and behaviors. Ideally only used for teen volunteers and trip participants. Adults should take all the required Safe Church courses.

Incident Report

In the event that something happens at your congregation or organization, this form can be used to keep track of who was involved and what actions were taken. It is best to fill the form out as soon as possible after all parties are cared for.

Field Trip Permission

If you are taking minors anywhere off-campus, you need to have written permission from their legal guardian. It is preferable to have these printed on paper and signed in ink. These forms can be edited for overnight events as well.

Minor Image Release

If you would like to use the image, video, or audio of a minor in your printed or online materials, you must have written permission from their legal guardian. It is preferable to have these printed on paper and signed in ink.

Volunteer Application

It is always best practice to ask new volunteers to be members of the congregation for at least 6 months and to have them complete an application. You should use this to conduct and interview and check references.

Volunteer Driver Information

Driving minors is a big responsibility, and not one to be trusted to just anyone. This form is to be used along with the Volunteer Application to cover as many bases as possible in securing the safety of the children and adults involved in your ministries.

If you have a concern you’d like to raise to the leadership of your congregation, school, or organization, this form is a sample of what should be included in that report. Leadership can help you ascertain if legal authorities should be contacted.
